Useful forms for training activities
If you undertake certain activities such as training, you will normally be required to complete certain forms. You can save yourself some time by printing them and completing them in advance. You'll find relevant forms below.
PADI Medical Statement - Required for all PADI training courses. You can fill this in yourself. If you answer "yes" to any questions, you must then have the form signed by a physician stating that you are fit to dive. A full doctor's examination is required for some countries.
PADI Safe Diving Practices Statement of Understanding - This is usually required before undertaking any PADI activity. This is not a liability release (which is not used in most European countries), but rather a statement that shows you understand what safe diving practices are, and that you agree to follow them.
